Ever heard of Dick’s Last Resort? Never been to the restaurant myself, but I love the concept. It’s like the court jester in medieval times being able to tell the King the harsh truth since everything he said was in—you guessed it—jest. Why has no pharmacy taken up this business practice? If patients expect harsh but fair truth from the pharmacy team, they’ll accept it readily.

Side Effects

[Intern]: “Hey, Roger, you play poker at all?” 
[Rph]: “Not since my pharmacy school years.  Why’s that?” 
[Intern]: “Doesn’t really matter.  I got a poker/pharmacy puzzle for you.  Ready?” 
[Rph]: “Kinda busy, but sure, go ahead.” 
[Intern]: “So I’m sitting right next to this guy who bets all-in on the river.  My hand’s not great, but if the guy’s bluffing, it should be good enough to win.” 
[Rph]: “Was this at the casino?” 
[Intern]: “It’s hypothetical.  Anyway, since he sat down an hour ago, he hasn’t stopped moving his left leg.  Constantly flexing it and rubbing it.  So I ask him, ‘You take anything for your restless leg syndrome?’  He answers: ‘Pramipexole.’  What do you do?” 
[Rph]: “Was this on one of your practice tests for the NAPLEX?”  
[Intern]: “I told you, it’s just hypothetical.” 
[Rph]: “I really have no idea what you’re even asking, Owen.” 
[Intern]: “You should call his bet because a side effect of Pramipexole could be impulse control disorder.  So there’s an increased chance he’s bluffing.” 
[Rph]: “You know, sleep attacks are another odd side effect.  Wait for him to fall asleep, then look at his cards.”   
[Intern]: “Orthostatic hypotension, too.  Do something to make him stand up really quick—threaten to kick his ass or something.  He passes out, then call the clock.” 
[Rph]: “Call the clock?” 
[Intern]: “Yeah.  If someone’s taking too long, you call the clock.  Then they get 60 more seconds to act, otherwise their hand is dead and you win the pot by default.”   
[Rph]: “Now that’s some real-world application of pharmacy knowledge!”
